Wanting to make a Referral?

If you would like to make a referral on behalf of someone you are or have been working with, please follow the instructions below.


The person has experienced domestic abuse
in the last 6 months

If the person you are or have been working with has disclosed they are a victim of domestic abuse and that there have been incidents of abuse (financial, emotional and psychological, sexual, physical, stalking and harassment and control and coercion) in the last 6 months please complete a DASH (Domestic Abuse Stalking and Harassment) Assessment (docx file). 

Important

Please ensure you have the client’s consent before sending us the referral and make this clear on the DASH.

How to complete a DASH Assessment

If you require any support when completing the DASH assessment with the person you are or have been supporting, please read the guidance on How to Use the DASH Risk Assessment (pdf file).

DASH-specific training offered

Alternatively, if you would like Rotherham Rise to provide your organisation with some training on domestic abuse or the DASH assessment, please email [email protected].

over 6 months ago

If the person you are or have been working with has disclosed they are a victim of domestic abuse but the last incident of abuse (financial, emotional and psychological, sexual, physical, stalking and harassment and control and coercion) was over 6 months ago, please complete our Referral Form (pdf file).

Once complete, please email this to [email protected]

Using the PDF form

Step 1. Download the Referral Form (pdf file).
Step 2. Open the form in Adobe Acrobat and complete it.
Step 3. Once you have completed it, including the required fields, select from the menu in Acrobat “Save As” with a new name for the file.
Step 4. Send your newly saved and completed form to [email protected].

What is the definition of domestic abuse?

The UK government’s definition of domestic abuse is:

“any incident or pattern of incidents of controlling, coercive, threatening behaviour, violence, or abuse between those aged 16 or over who are, or have been, intimate partners or family members regardless of gender or sexuality. The abuse can encompass, but is not limited to, psychological, physical, sexual, financial, emotional.”
